Log in to see your account balance, claim activity, eligible expenses and much more!

Submit a claim online or complete a Claim Form

•Include copies of your itemized receipt or Explanation of Benefits (EOB) that shows:
 

-Date expense was incurred

-Amount of expense

-Description of the service or item you bought
 

•Review your documentation and your reimbursement form
for completeness

•Be sure to sign and date your form

Please note: cancelled checks are not acceptable.
